What is a Home Care Package?
Home Care Packages are subsidies that have been designed by the government to help older Australians remain independent in their own home for as long as they can.
If you are eligible for a Home Care Package, the Australian Government will provide you with a subsidy. This subsidy allows you to select a provider of your choice to deliver services and provide Case Management to meet your individual needs.
There are four levels of Home Care Packages:
Basic - Cleaning, gardening, transport, social support or help with meal preparation.
Low - Same as level 1, but with a nurse to visit and assist with medication of wound management & wound care.
Intermediate - May include middle-level health care, such as catheter or continence management, assistance with mobility equipment such as walking frames and bed rails. Support from level 1 & 2 is included as well.
High - Includes high-level care such as 24-hour support from medical staff, assistance with feeding, enteral feeding formula and equipment or allied referrals.
Candice Care provides care and support on all four levels.
Your Home Care Package Level will be determined depending on your needs and will need to be approved by assigned government bodies.
How do I get a Home Care Package?
You may be eligible for a Home Care Package if you are:
- An older person living in Australia who needs coordinated support to help you stay in your home
Home Care Packages are meant for older people, but there is no minimum age requirement. There are no citizenship or residency restrictions, however the subsidies are not meant for visitors to Australia or people who only need temporary/short-term care.
In order to get a Home Care Package, you or your family member needs to be assessed by the My Aged Care team and the Aged Care Assessment Team (ACAT).
To get started, follow the steps below:
After your application, My Aged Care will contact you to arrange a face-to-face inspection. At this assessment, your exact needs will be determined.
After the assessment, My Aged Care will let you know what services you are eligible for.
